1. Purpose of the Policy
This confidentiality policy outlines how I, Tiffany Leung, Chartered Psychologist, manage, protect, and use personal data and client information in accordance with the Data Protection Act 2018 and the General Data Protection Regulation (GDPR). Your confidentiality and privacy are of the utmost importance to me, and this policy explains your rights and my responsibilities regarding the information you share with me.
2. Confidentiality Commitment
I am committed to ensuring that any information you provide during our sessions is handled with the strictest confidentiality. This includes, but is not limited to:

I will only collect and use your personal information for the purpose of providing psychological services. The data collected may include:

The information collected is used to provide effective treatment, manage appointments, and comply with any legal or professional requirements.

4. Limits of Confidentiality
While I strive to maintain the confidentiality of all information shared in our sessions, there are certain circumstances under which I may be legally or ethically required to break confidentiality:

Harm to Self or Others: If I believe you are at serious risk of harming yourself or others, I am required to take steps to prevent harm, which may involve contacting relevant authorities or individuals.

Legal Obligations: If I am ordered by a court or required by law to disclose information, I may need to share certain details without your consent.

Safeguarding Children or Vulnerable Adults: If there are concerns about the safety of a child or vulnerable adult, I may be required to disclose information to protect them from harm.
In all of the above situations, I will make every effort to discuss any proposed breach of confidentiality with you prior to disclosure, unless doing so would increase the risk of harm.

5. Storage and Security of Information
All client information is stored securely to protect your privacy. I use secure, password-protected electronic systems to store records and session notes. Any paper records are stored in locked, secure locations. Information is retained in accordance with legal and professional guidelines and is only accessed by myself.
I regularly review my data security procedures to ensure they meet current standards. If any data breach occurs that may affect your confidentiality, you will be informed immediately.

6. Sharing Information with Third Parties
I will not share your information with third parties without your explicit consent, except in the circumstances outlined in Section 4 of this policy.
If you consent, I may liaise with other healthcare professionals (such as your GP) or relevant third parties to ensure that you receive the best care possible. However, this will only be done with your permission, and you will be kept informed of any communication.

7. Client Access to Information
You have the right to request access to any personal information I hold about you. If you wish to view or receive a copy of your records, please submit a written request to me via email at corimuscoun@gmail.com.
I aim to respond to access requests within 30 days, in accordance with GDPR requirements.

8. Retention of Records
In compliance with professional and legal requirements, I will retain your records for a minimum of seven years after the conclusion of therapy. For clients under 18, records will be retained until they reach the age of 25, in line with UK legislation.
Once this retention period is over, your information will be securely destroyed or deleted.

9. Complaints and Concerns
If you have any concerns or complaints about how your information is being handled, please feel free to discuss these with me. I take confidentiality and data protection seriously, and I will work with you to resolve any issues.
If you are not satisfied with the response, you have the right to lodge a complaint with the Information Commissioner's Office (ICO), which regulates data protection in the UK.

10. Policy Review
